At DJS Psychiatry, we strive to make the psychiatric care referral process as seamless as possible for healthcare providers and patients. We understand the importance of timely access to mental health services and are dedicated to providing compassionate, expert care for all referred patients.

How to Refer a Patient

Our referral process is straightforward, allowing healthcare providers to efficiently connect their patients with our team of experienced mental health professionals. To refer a patient to DJS Psychiatry:

  1. Submit a Referral Form: Please fill out our Referral Form with the necessary patient details and relevant medical history.
  2. Send Medical Records: Ensure all pertinent medical and psychiatric records are included to help our team develop an appropriate care plan.
  3. Confirmation of Referral: After submitting the referral, you will receive a confirmation email, and our team will reach out to the patient within 48 hours to schedule an appointment.
