Please provide complete and accurate information. Missing forms, insurance information or required clinical records may delay scheduling or confirmation of an appointment.
Submit Registration
Tell us who the patient is, how to contact you, the reason for seeking care, your preferred service and whether you are requesting telehealth or an in-office visit.
Provide Insurance & Payment Information
Submit current insurance details when using insurance. Our team may review eligibility and benefits; coverage is ultimately determined by your health plan. Self-pay arrangements can also be discussed when applicable.
Complete Clinical Intake Forms
Complete the requested psychiatric history, medical history, current medication list, allergies, prior treatment history, family and social history, screening questionnaires, consent forms and practice policies.
Send Supporting Records When Requested
Depending on the reason for referral, we may request prior psychiatric records, hospital or discharge summaries, medication history, psychological or neuropsychological testing, school information, laboratory results or relevant medical records.
Clinical & Administrative Review
Our team reviews the information to determine the appropriate clinician, service, visit type and whether the practice can meet the requested level of care.
Appointment Confirmation
Once the necessary registration and intake items are complete, the office will provide available scheduling options and instructions for your telehealth or in-office appointment.

For minors, include legal guardian and custody information when applicable.
Current insurance card details and subscriber information.
Current medications, doses, prescribers, allergies and previous psychiatric medication trials.
Previous diagnoses, therapy, psychiatric care, hospitalizations and relevant testing.
Main symptoms, concerns, goals and any recent changes affecting functioning.
